The certification that expired three months ago
A monitor arrives for a pre-activation review. The essential records checklist looks strong--IRB approval is in hand, the delegation log is signed, investigator CVs are current. Then the monitor asks for the laboratory certification. The coordinator pulls the CLIA certificate from the file. The monitor glances at the expiration date, pauses, and says: "This expired in December."
