
Safety reporting metrics: tracking timeliness, completeness, and compliance across the portfolio
Design a safety reporting metrics dashboard with quality tolerance limits for each KPI, analyze performance trends to distinguish isolated lapses from systemic failures, and evaluate which metrics are leading indicators that provide time to act versus lagging indicators that document what already went wrong.
What gets measured gets managed -- but only if you measure the right things
There is a management maxim, usually attributed to Peter Drucker, that what gets measured gets managed. It is cited so frequently in quality management circles that it has become almost meaningless through repetition. But in safety reporting coordination, the principle has teeth -- and a corollary that matters more than the original: measuring the wrong things creates the illusion of control without the substance of it.
A regulatory coordinator who tracks only whether safety reports were submitted on time is measuring the most obvious metric while missing the most important ones. By the time a report is late, the failure has already occurred. The deadline was missed. The sponsor made safety decisions without your site's data. The IRB evaluated continuing risk without your submission. Measuring late reports tells you what went wrong. It does not tell you what is about to go wrong.
In the previous two lessons, you built the infrastructure that makes portfolio-level safety reporting visible: the calendar that maps every deadline across every study, and the cross-study trend detection framework that identifies safety patterns invisible within any single protocol. This lesson adds the quantitative layer. You will design the metrics that tell you whether your safety reporting pipeline is healthy, whether its performance is stable or deteriorating, and -- most importantly -- whether the early warning signs of a future failure are present in today's data.
ICH E6(R3), Section 3.10 requires that the sponsor implement a quality management system incorporating risk-based approaches and quality tolerance limits. While that provision applies directly to sponsors, the principle it embodies -- that systematic measurement prevents quality failures -- applies with equal force at the investigator site. The RC who builds a metrics dashboard is not satisfying a regulatory requirement directed at someone else. The RC is building the operational intelligence that prevents the site's safety reporting pipeline from failing silently.
