I want to be honest about something I have observed repeatedly in clinical research: many sites complete CAPA plans as a compliance exercise. The plan is written because the sponsor or the IRB requires it, not because the site intends to implement meaningful change. The corrective action is "retraining" — a single conversation with the coordinator who made the error. The preventive action is "increased oversight" — a vague commitment to pay more attention. Neither of these addresses the root cause. Neither of these prevents recurrence. And six months later, the same deviation occurs again, and the cycle repeats.
