The moment source meets CRF
There is a moment during every monitoring visit that defines the relationship between a coordinator and the data they have entered. The monitor opens the source document -- a vital signs worksheet, a laboratory printout, an electronic medical record note -- and places it beside the CRF. Then they look. Value by value, field by field, they compare what the source says to what the coordinator entered. This is source data verification, and it is one of the most consequential quality control activities in a clinical trial.
