The assessment that was finished -- until it was not
The amendment looked straightforward. A Phase III diabetes trial changed the fasting glucose collection window from "10β14 hours fasting" to "8β12 hours fasting." One line in the protocol. The regulatory coordinator completed the impact assessment in under an hour: IRB submission required, consent form unaffected, source documents need a minor revision to the fasting window notation. Three items. Done.
Except it was not done. The coordinator had overlooked three operational domains. The laboratory needed updated specimen handling instructions because the narrower fasting window changed the acceptable range for quality control flagging. Participant instruction sheets -- the printed cards sent home with participants explaining pre-visit preparation -- referenced the old 10β14 hour window and needed reprinting. And the CRC team needed documented retraining on the new window because they were the ones verifying fasting compliance at check-in, and three different coordinators had three different understandings of whether "8 hours" meant "at least 8" or "exactly 8."
The monitor found all three gaps at the next visit. Not because the coordinator was careless -- because the coordinator did not have a systematic tool that forced evaluation of every operational domain. The analytical skill taught in M1-L2 identifies what an amendment changes. The classification system in M1-L3 determines the regulatory pathway. This lesson gives you the template that ensures you scope the complete work -- every domain, every dependency, every resource requirement -- before declaring the assessment finished.

The template forces completeness regardless of who completes it, how many amendments arrived that week, or whether the coordinator is running on four hours of sleep. And when the monitor asks to see the site's assessment of Amendment 6, the template is the document that demonstrates the site evaluated every domain -- not a mental recollection that the coordinator "thought about it."
I want to note something about the evolution from M1-L2's five domains to this lesson's eight. The five-domain framework is an analytical scanning tool -- broad categories designed to help the coordinator identify consequences quickly. The eight-domain impact assessment template is an operational documentation tool -- specific functional areas that map to the people and processes that must act on those consequences. The five feed into the eight. They are not competing frameworks; they are different instruments for different stages of the same workflow.
ICH E6(R3) Section 2.2.2: Adequate resources for trial conduct
Section 2.2.2 requires the investigator to have "adequate resources for the proposed trial," including "adequate time, appropriately qualified staff... and adequate facilities for the foreseen duration of the trial." An impact assessment that fails to identify the resource requirements of an amendment directly undermines the investigator's ability to meet this obligation. The template is not merely an organizational convenience. It is the mechanism by which the site determines whether it has the staff, time, and infrastructure to implement the amendment completely.
The eight operational domains
The impact assessment template evaluates each amendment against eight operational domains. These are not arbitrary divisions -- they map to the functional areas of a clinical research site where amendment-driven changes must be executed. Each domain has a responsible party, a documentation requirement, and a regulatory anchor.
Notice that domains 2 and 8 -- informed consent and participant communication -- are distinct. This is deliberate. The consent form is a legal document requiring IRB approval before use. Participant instruction sheets, visit calendars, and preparation guides are operational materials that may not require separate IRB approval but must accurately reflect the current protocol. An amendment that changes fasting requirements may not require a consent revision -- fasting duration is rarely a consent-worthy risk disclosure -- but it absolutely requires updated participant instruction materials. Conflating these two domains is how the coordinator in the opening scenario missed the participant instruction sheets entirely.
Building the template: structure and workflow
The impact assessment template is a single document completed for every amendment. It captures three things: which domains are affected, what specific actions are required within each affected domain, and what resources and timelines those actions demand. The structure is straightforward, but the discipline of completing it systematically -- domain by domain, without skipping -- is what prevents gaps.
Here is how the template works in practice. The coordinator receives an amendment, logs it through the intake process (M1-L1), reads it for operational impact using the five-domain scan (M1-L2), and classifies it into a regulatory category (M1-L3). Then the coordinator opens the impact assessment template and works through each of the eight domains in sequence. For each domain, three questions must be answered:

1. Is this domain affected? Not every amendment touches every domain. A change to the medical monitor's contact information affects regulatory submissions (updated protocol page in the binder) but does not affect pharmacy, laboratory, or source documents. The template requires the coordinator to explicitly evaluate each domain and document "not affected" where applicable -- because a blank entry is ambiguous, while "not affected" is a documented determination.
2. What specific actions are required? For each affected domain, the coordinator lists the concrete tasks: "Revise source document worksheet SD-04 to reflect new fasting window," "Submit revised consent form to IRB with tracked changes," "Schedule CRC retraining session before next participant visit." Specificity matters. "Update source documents" is insufficient. Which source documents? What revision? Who is responsible?
3. What resources and timeline does this require? Each action has a resource requirement (who does the work, how long does it take) and a timeline dependency (when must it be completed, and what must happen before it can begin). A consent form revision cannot be submitted to the IRB until the investigator reviews and approves it. Staff retraining cannot occur until the revised source documents are available. These dependencies determine the sequencing of the implementation plan.
Determining resource requirements and timeline dependencies
The impact assessment is not merely a checklist of affected domains. It is a scoping document that answers the question every investigator and site director needs answered: How much work does this amendment create, and how long will it take?
Resource requirements fall into three categories. Staff time is the most consistently underestimated: revising source documents, drafting consent language, scheduling and conducting training sessions, reprinting participant materials, and coordinating with the pharmacy and laboratory all consume hours that must be allocated from a team already managing its existing workload. External coordination captures the dependencies that are not within the site's control: the sponsor must deploy EDC updates, the IRB must review and approve, the central laboratory must ship new kits. Infrastructure captures physical and system requirements: new specimen collection tubes in stock, updated forms printed and distributed, EDC access reconfigured.
Timeline dependencies are the sequencing constraints that determine the critical path of implementation. Some dependencies are regulatory: the site cannot implement a substantive amendment until the IRB approves it. Some are logical: the coordinator cannot train staff on revised procedures until the revised source documents exist. And some are external: the site cannot begin collecting specimens with new tubes until the laboratory ships them.
Applying the assessment across concurrent studies
When three amendments arrive in the same week across different studies -- and they will -- the impact assessment template for each amendment produces a resource and timeline picture that the coordinator can overlay. If Amendment A requires 12 staff-hours, Amendment B requires 8, and Amendment C requires 15, the coordinator knows the team faces 35 hours of amendment work in addition to the ongoing study conduct workload. This is not triage (that is M5
L2) and not resource allocation during surges (that is M5
L3). This is the scoping input that makes triage and allocation possible. Without a quantified assessment of each amendment's resource demands, the coordinator is guessing at priorities.
The Appendix B cross-reference: validating completeness
The eight-domain template is a strong tool. But even a strong tool has a weakness: it depends on the coordinator recognizing that a domain is affected. If the coordinator does not realize that a fasting window change affects the laboratory domain, the template will show "Domain 7: Not affected" -- a documented but incorrect determination.
This is where the Appendix B cross-reference becomes indispensable. ICH E6(R3) Appendix B defines the required content elements of a clinical trial protocol, organized into 16 sections (B.1 through B.16). Each section corresponds to a category of protocol content that may be modified by an amendment. By cross-referencing the amendment against the Appendix B sections, the coordinator can identify affected protocol elements that might not have been obvious from reading the amendment text alone.
The method is direct. After completing the eight-domain assessment, the coordinator takes the amendment's change summary and maps each change to the Appendix B section it modifies. Then the coordinator asks: Does the Appendix B section I have identified connect to any domain I marked as "not affected"?

The Appendix B cross-reference is not a replacement for the eight-domain assessment. It is a validation layer. The assessment captures what the coordinator identified. The cross-reference catches what the coordinator missed. In my experience, the cross-reference routinely reveals at least one missed domain -- often enough that I consider it indispensable rather than optional. Every domain caught at this stage is an implementation gap that would otherwise surface during monitoring.
There is a subtlety worth flagging. Not every Appendix B section maps neatly to a single domain. B.4.6 -- the schedule of events -- touches five of the eight domains. A change to the visit schedule is, operationally, an amendment that affects nearly everything. The cross-reference reveals that breadth. Without it, a coordinator might revise the source documents and update the EDC but forget to reprint participant visit calendars or retrain staff on the new visit windows.
Appendix C: Essential records as a secondary check
ICH E6(R3) Appendix C provides the Essential Records Table -- a catalog of the documents that must be maintained during a clinical trial. After completing the impact assessment and the Appendix B cross-reference, a brief scan of Appendix C confirms that every document affected by the amendment is accounted for in the implementation plan. If the amendment changes a procedure, the Appendix C check asks: "Is the source document for that procedure listed in my assessment? Is the training record? Is the investigator's signed agreement?" This is a 5-minute verification that catches documentation gaps before they become audit findings.
The assessment output: from scoping to handoff
The completed impact assessment produces a document with four sections. Together, these sections constitute the deliverable that the coordinator uses to brief the investigator, communicate the implementation scope to the CRC team, and -- critically -- demonstrate to the monitor that the site evaluated the amendment systematically.
Section 1: Amendment identification. The protocol, amendment number, date received, classification (from M1-L3), and regulatory pathway. This header connects the assessment to the specific amendment and its regulatory context.
Section 2: Domain-by-domain evaluation. For each of the eight domains: affected or not affected, specific actions required, responsible party, and timeline. Domains marked "not affected" include a brief rationale -- "Pharmacy not affected: amendment does not modify dosing, drug supply, or storage requirements."
Section 3: Resource summary. Total staff-hours estimated, external dependencies identified, and infrastructure requirements noted. This section answers the investigator's question: "What does this cost us?"
Section 4: Timeline and dependencies. A sequenced implementation plan showing which actions must be completed first (regulatory submissions, investigator review), which can proceed in parallel (source document revision, participant material reprinting), and which depend on external inputs (EDC deployment, laboratory kit shipment). The critical path -- the longest sequence of dependent actions -- determines the earliest date the amendment can be fully implemented.
This assessment produces scoping output, not implementation mapping
The impact assessment answers "What work does this amendment create?" It does not answer "How do we track every cascading change through to completion?" That is the implementation cascade, which you will build in M4
L1. The distinction is important: scoping tells you the size and shape of the work. Implementation mapping tells you how to execute and verify every step. Confusing the two leads to either an assessment that tries to be an implementation tracker (too detailed, too slow) or an implementation plan that skips the scoping step (incomplete, reactive). The assessment comes first. The cascade builds on it.
Putting the template to work
The methodology is clear: eight domains, three questions per domain, Appendix B cross-reference, structured output. But the value of the template becomes most apparent when it catches something the coordinator would otherwise have missed -- when the systematic process reveals a gap that analytical intuition alone did not surface.

The amendment modifies the fasting requirement, which falls under Appendix B, Section B.4.6 (schedule of events and assessments) and B.9 (assessment of safety). She maps B.4.6 to her domain table: source documents (already captured), data collection (already captured), laboratory, participant communication, and staff training. She maps B.9: laboratory, source documents, data collection.
Two domains she marked "not affected" -- laboratory and participant communication -- appear in the mapping. One domain she marked "not affected" -- staff training -- also appears.
Nadia returns to each domain.
Laboratory (revised): Affected. The central laboratory's quality control flagging criteria are calibrated to the 10β14 hour fasting window. Specimens collected after only 8 hours of fasting may produce glucose values that trigger QC flags under the old parameters. Nadia must confirm with the laboratory whether the QC reference ranges need updating and whether new specimen handling instructions will be issued.
Participant communication (revised): Affected. Lakeshore distributes printed pre-visit instruction cards to CLARITY-4 participants. The current cards state: "Do not eat or drink anything except water for at least 10 hours before your study visit." These cards must be reprinted with the updated 8-hour instruction and redistributed to all 62 enrolled participants before their next fasting visit.
Staff training (revised): Affected. Three CRCs conduct fasting verification at participant check-in. The verification question -- "When did you last eat?" -- has not changed, but the acceptable answer has. Under the old window, a participant who last ate at 8 PM and arrives at 7 AM (11 hours fasting) was compliant. Under the new window, a participant who last ate at 11 PM and arrives at 7 AM (8 hours fasting) is also compliant -- but two of the three CRCs were trained under the old parameters and may question or flag a participant who fasted only 8 hours. The retraining takes 15 minutes but must be documented per Section 2.3.2.
The revised assessment: Six domains affected, not three. The resource estimate increases from 4 staff-hours to approximately 9. The timeline extends to 14 business days to accommodate the laboratory QC confirmation and participant card reprinting.
The challenge:
Without the Appendix B cross-reference, Nadia's assessment would have been submitted with three domains missing. The laboratory QC gap would have surfaced when the first post-amendment specimen triggered a flag the coordinator could not explain. The participant instruction gap would have surfaced when a participant arrived with 9 hours of fasting and was told by a CRC that the minimum was 10. The training gap would have surfaced when the monitor asked for documentation of CRC retraining and found none.

Without a thorough impact assessment, every subsequent step is built on incomplete information. And incomplete information, in clinical research, does not produce harmless oversights. It produces the implementation gaps that monitors discover, that auditors cite, and that -- at their worst -- affect the data integrity and participant protections the site exists to safeguard.

Each action has a resource requirement (who does the work, how long does it take) and a timeline dependency (when must it be completed, and what must happen before it can begin). A consent form revision cannot be submitted to the IRB until the investigator reviews and approves it. Staff retraining cannot occur until the revised source documents are available. These dependencies determine the sequencing of the implementation plan.
Determining resource requirements and timeline dependencies
The impact assessment is not merely a checklist of affected domains. It is a scoping document that answers the question every investigator and site director needs answered: How much work does this amendment create, and how long will it take?
Resource requirements fall into three categories. Staff time is the most consistently underestimated: revising source documents, drafting consent language, scheduling and conducting training sessions, reprinting participant materials, and coordinating with the pharmacy and laboratory all consume hours that must be allocated from a team already managing its existing workload. External coordination captures the dependencies that are not within the site's control: the sponsor must deploy EDC updates, the IRB must review and approve, the central laboratory must ship new kits. Infrastructure captures physical and system requirements: new specimen collection tubes in stock, updated forms printed and distributed, EDC access reconfigured.
Timeline dependencies are the sequencing constraints that determine the critical path of implementation. Some dependencies are regulatory: the site cannot implement a substantive amendment until the IRB approves it. Some are logical: the coordinator cannot train staff on revised procedures until the revised source documents exist. And some are external: the site cannot begin collecting specimens with new tubes until the laboratory ships them.
Applying the assessment across concurrent studies
When three amendments arrive in the same week across different studies -- and they will -- the impact assessment template for each amendment produces a resource and timeline picture that the coordinator can overlay. If Amendment A requires 12 staff-hours, Amendment B requires 8, and Amendment C requires 15, the coordinator knows the team faces 35 hours of amendment work in addition to the ongoing study conduct workload. This is not triage (that is M5
L2) and not resource allocation during surges (that is M5
L3). This is the scoping input that makes triage and allocation possible. Without a quantified assessment of each amendment's resource demands, the coordinator is guessing at priorities.
The Appendix B cross-reference: validating completeness
The eight-domain template is a strong tool. But even a strong tool has a weakness: it depends on the coordinator recognizing that a domain is affected. If the coordinator does not realize that a fasting window change affects the laboratory domain, the template will show "Domain 7: Not affected" -- a documented but incorrect determination.
This is where the Appendix B cross-reference becomes indispensable. ICH E6(R3) Appendix B defines the required content elements of a clinical trial protocol, organized into 16 sections (B.1 through B.16). Each section corresponds to a category of protocol content that may be modified by an amendment. By cross-referencing the amendment against the Appendix B sections, the coordinator can identify affected protocol elements that might not have been obvious from reading the amendment text alone.
The method is direct. After completing the eight-domain assessment, the coordinator takes the amendment's change summary and maps each change to the Appendix B section it modifies. Then the coordinator asks: Does the Appendix B section I have identified connect to any domain I marked as "not affected"?

The Appendix B cross-reference is not a replacement for the eight-domain assessment. It is a validation layer. The assessment captures what the coordinator identified. The cross-reference catches what the coordinator missed. In my experience, the cross-reference routinely reveals at least one missed domain -- often enough that I consider it indispensable rather than optional. Every domain caught at this stage is an implementation gap that would otherwise surface during monitoring.
There is a subtlety worth flagging. Not every Appendix B section maps neatly to a single domain. B.4.6 -- the schedule of events -- touches five of the eight domains. A change to the visit schedule is, operationally, an amendment that affects nearly everything. The cross-reference reveals that breadth. Without it, a coordinator might revise the source documents and update the EDC but forget to reprint participant visit calendars or retrain staff on the new visit windows.
Appendix C: Essential records as a secondary check
ICH E6(R3) Appendix C provides the Essential Records Table -- a catalog of the documents that must be maintained during a clinical trial. After completing the impact assessment and the Appendix B cross-reference, a brief scan of Appendix C confirms that every document affected by the amendment is accounted for in the implementation plan. If the amendment changes a procedure, the Appendix C check asks: "Is the source document for that procedure listed in my assessment? Is the training record? Is the investigator's signed agreement?" This is a 5-minute verification that catches documentation gaps before they become audit findings.
The assessment output: from scoping to handoff
The completed impact assessment produces a document with four sections. Together, these sections constitute the deliverable that the coordinator uses to brief the investigator, communicate the implementation scope to the CRC team, and -- critically -- demonstrate to the monitor that the site evaluated the amendment systematically.
Section 1: Amendment identification. The protocol, amendment number, date received, classification (from M1-L3), and regulatory pathway. This header connects the assessment to the specific amendment and its regulatory context.
Section 2: Domain-by-domain evaluation. For each of the eight domains: affected or not affected, specific actions required, responsible party, and timeline. Domains marked "not affected" include a brief rationale -- "Pharmacy not affected: amendment does not modify dosing, drug supply, or storage requirements."
Section 3: Resource summary. Total staff-hours estimated, external dependencies identified, and infrastructure requirements noted. This section answers the investigator's question: "What does this cost us?"
Section 4: Timeline and dependencies. A sequenced implementation plan showing which actions must be completed first (regulatory submissions, investigator review), which can proceed in parallel (source document revision, participant material reprinting), and which depend on external inputs (EDC deployment, laboratory kit shipment). The critical path -- the longest sequence of dependent actions -- determines the earliest date the amendment can be fully implemented.
This assessment produces scoping output, not implementation mapping
The impact assessment answers "What work does this amendment create?" It does not answer "How do we track every cascading change through to completion?" That is the implementation cascade, which you will build in M4
L1. The distinction is important: scoping tells you the size and shape of the work. Implementation mapping tells you how to execute and verify every step. Confusing the two leads to either an assessment that tries to be an implementation tracker (too detailed, too slow) or an implementation plan that skips the scoping step (incomplete, reactive). The assessment comes first. The cascade builds on it.
Putting the template to work
The methodology is clear: eight domains, three questions per domain, Appendix B cross-reference, structured output. But the value of the template becomes most apparent when it catches something the coordinator would otherwise have missed -- when the systematic process reveals a gap that analytical intuition alone did not surface.
The following case study illustrates exactly that moment.

Regulatory submissions: Affected. The amended protocol must be submitted to Lakeshore's institutional IRB. The investigator signature page must be re-executed.
Informed consent: Not affected. The fasting window is not a risk disclosure element -- the change reduces participant burden, and the consent form does not specify the fasting duration in hours.
Staff training: Not affected. "The CRCs already know how to verify fasting compliance -- the window just shifts."
Source documents: Affected. The source document worksheet that records fasting verification must be updated from "10β14 hours" to "8β12 hours."
Data collection tools: Affected. The EDC field for fasting duration verification must be updated by the sponsor, and Nadia must confirm the deployment timeline with the CRA.
Domains 6β8 (initially marked "not affected"):
Pharmacy: Not affected.
Laboratory: Not affected.
Participant communication: Not affected.
Nadia's assessment takes 40 minutes. Three domains affected, five not affected. She drafts the resource summary: approximately 4 staff-hours total. Implementation timeline: 10 business days, contingent on IRB approval.
Then she runs the Appendix B cross-reference.
The amendment modifies the fasting requirement, which falls under Appendix B, Section B.4.6 (schedule of events and assessments) and B.9 (assessment of safety). She maps B.4.6 to her domain table: source documents (already captured), data collection (already captured), laboratory, participant communication, and staff training. She maps B.9: laboratory, source documents, data collection.
Two domains she marked "not affected" -- laboratory and participant communication -- appear in the mapping. One domain she marked "not affected" -- staff training -- also appears.
Nadia returns to each domain.
Laboratory (revised): Affected. The central laboratory's quality control flagging criteria are calibrated to the 10β14 hour fasting window. Specimens collected after only 8 hours of fasting may produce glucose values that trigger QC flags under the old parameters. Nadia must confirm with the laboratory whether the QC reference ranges need updating and whether new specimen handling instructions will be issued.
Participant communication (revised): Affected. Lakeshore distributes printed pre-visit instruction cards to CLARITY-4 participants. The current cards state: "Do not eat or drink anything except water for at least 10 hours before your study visit." These cards must be reprinted with the updated 8-hour instruction and redistributed to all 62 enrolled participants before their next fasting visit.
Staff training (revised): Affected. Three CRCs conduct fasting verification at participant check-in. The verification question -- "When did you last eat?" -- has not changed, but the acceptable answer has. Under the old window, a participant who last ate at 8 PM and arrives at 7 AM (11 hours fasting) was compliant. Under the new window, a participant who last ate at 11 PM and arrives at 7 AM (8 hours fasting) is also compliant -- but two of the three CRCs were trained under the old parameters and may question or flag a participant who fasted only 8 hours. The retraining takes 15 minutes but must be documented per Section 2.3.2.
The revised assessment: Six domains affected, not three. The resource estimate increases from 4 staff-hours to approximately 9. The timeline extends to 14 business days to accommodate the laboratory QC confirmation and participant card reprinting.
The challenge:
Without the Appendix B cross-reference, Nadia's assessment would have been submitted with three domains missing. The laboratory QC gap would have surfaced when the first post-amendment specimen triggered a flag the coordinator could not explain. The participant instruction gap would have surfaced when a participant arrived with 9 hours of fasting and was told by a CRC that the minimum was 10. The training gap would have surfaced when the monitor asked for documentation of CRC retraining and found none.
Analysis

M1-L1 (Intake): The amendment arrives. The coordinator logs it, timestamps receipt, and routes it for review. The regulatory clock starts.
M1-L2 (Operational impact): The coordinator reads the amendment as an operational analyst, scanning across five domains, sorting consequences by urgency, and distinguishing enrolled from future participants. This is the analytical thinking.
M1-L3 (Classification): The coordinator classifies the amendment into a regulatory category -- administrative, substantive, or safety-driven -- which determines the IRB pathway, the implementation timeline, and whether the amendment may be implemented before approval.

The output of this sequence is a documented, validated assessment that tells the coordinator -- and the entire site team -- exactly what the amendment requires: the regulatory pathway, the scope of the work, the resources needed, the timeline constraints, and the sequencing dependencies. This assessment is the input for everything that follows: the IRB submission (Module 2), the consent revision and reconsent plan (Module 3), the implementation cascade (Module 4), and the portfolio-level prioritization (Module 5).
Without a thorough impact assessment, every subsequent step is built on incomplete information. And incomplete information, in clinical research, does not produce harmless oversights. It produces the implementation gaps that monitors discover, that auditors cite, and that -- at their worst -- affect the data integrity and participant protections the site exists to safeguard.
