It is 6:15 AM on a Wednesday in mid-March, and Sarah Mitchell is reviewing her preparation notes in the lobby of the Hampton Inn in Minneapolis, Minnesota. In three hours, she will arrive at Lakewood Regional Medical Center for her fourth monitoring visit to Site 108 in the ELEVATE-HF trial. Her laptop displays the previous monitoring visit report, dated eight weeks ago, along with a spreadsheet of open action items that should have been resolved since then.

I want to be direct about something that newer monitoring professionals sometimes miss: the documentation you create during monitoring activities is not primarily for your sponsor's files. It is evidence. Evidence that participants were protected. Evidence that data quality was verified. Evidence that problems were identified and corrected. When you write a monitoring visit report, you are writing for an inspector who may read it five years from now, asking whether this sponsor took its oversight obligations seriously.
