Stomach ulcers: For most of the twentieth century, ulcers were attributed to stress and treated with antacids, bland diets, and in severe cases, surgery. When researchers proposed that a bacterium called Helicobacter pylori caused most ulcers, the medical establishment was skeptical. Proper clinical trials demonstrated that antibiotics, not antacids, cured ulcers. This discovery transformed treatment and won its discoverers the Nobel Prize.
Back surgery: Certain spinal surgeries were performed for decades based on the assumption that they relieved pain by correcting structural problems. When researchers conducted sham-surgery trials, where patients underwent an incision but no actual procedure, they found that sham surgery often worked as well as the real procedure. The surgery's benefit was largely placebo.
These examples illustrate a humbling truth: without rigorous testing, physicians cannot reliably distinguish effective treatments from ineffective ones. Our intuitions mislead us. Our observations are biased. Our reasoning is flawed. Only systematic evidence can cut through the noise.
Evidence-based medicine (EBM) is the conscientious, explicit, and judicious use of current best evidence in making decisions about the care of individual patients. It integrates individual clinical expertise with the best available external clinical evidence from systematic research.

Not all evidence is created equal. Medical researchers have developed a hierarchy that ranks different types of evidence by their reliability in determining whether treatments work. Understanding this hierarchy is fundamental to understanding why clinical research matters.
From weakest to strongest
Each level represents a different degree of evidence of this hierarchy, explaining what it means and why some forms of evidence are more trustworthy than others.
Expert opinion and anecdotal evidence sit at the bottom. This includes what experienced physicians believe based on their practice, case reports describing individual patients, and testimonials from people who tried a treatment. This evidence is important for generating hypotheses, but it cannot establish whether treatments work. Experts have been wrong throughout medical history, and individual cases cannot account for the confounding factors we discussed earlier.
Case-control studies compare people who have a disease (cases) with people who do not (controls), looking backward to see what exposures or treatments differed between the groups. These studies are useful for identifying risk factors but are prone to recall bias (people with disease may remember exposures differently) and cannot establish causation.
Cohort studies follow groups of people over time, comparing those who receive a treatment or have an exposure to those who do not. These are more reliable than case-control studies because they observe events as they unfold, but they still cannot account for differences between the groups that might explain different outcomes.
Randomized controlled trials (RCTs) are the workhorse of clinical research. By randomly assigning participants to receive either the treatment under study or a comparison (often a placebo or standard treatment), RCTs ensure that the groups are similar in all ways except the treatment received. When properly conducted, RCTs can establish causation: if the treatment group does better, the treatment likely caused the improvement.
Systematic reviews and meta-analyses sit at the apex. These combine the results of multiple RCTs, increasing statistical power and providing a comprehensive view of all available evidence. A single trial might produce misleading results due to chance; when multiple trials agree, we can be more confident in the conclusions.

Why randomized trials matter
Randomization is the key innovation that makes modern clinical research possible. Let me explain why.
Imagine you want to test whether a new blood pressure medication works. You could simply give it to patients with high blood pressure and see if their blood pressure drops. But as we have discussed, blood pressure fluctuates naturally. Patients who seek treatment often have unusually high readings. They might change their diet or exercise more because they know they are being watched. The placebo effect might lower blood pressure on its own. Without a comparison group, you cannot know whether any improvement was due to the medication.
So you add a comparison group. Some patients receive the new medication; others receive the current standard treatment or a placebo. Now you can compare outcomes. But a new problem emerges: what if the patients in the medication group were healthier to begin with? What if they had less severe hypertension, or were younger, or had fewer other health problems? Any difference in outcomes might be due to these baseline differences, not the medication.
This is where randomization comes in. When you randomly assign patients to groups, the process is like shuffling a deck of cards. On average, all the characteristics that might affect outcomes, age, disease severity, other medications, diet, genetics, factors we have not even thought of, will be distributed equally between the groups. The only systematic difference will be the treatment received.
If, after randomization, the treatment group has lower blood pressure than the control group, and the difference is large enough that it is unlikely to have occurred by chance, we can conclude that the treatment caused the improvement. Randomization is what allows us to make causal claims rather than merely observational ones.
Randomization accomplishes something remarkable: it controls for every factor that might influence outcomes, including factors we do not know about or cannot measure.
Without randomization, we can only control for factors we think to measure. With randomization, we control for everything, known and unknown, measured and unmeasured. This is why randomized controlled trials are considered the gold standard for determining whether treatments work.
Evidence in action: how clinical research saves lives
Consider childhood leukemia. In the 1960s, the five-year survival rate for acute lymphoblastic leukemia in children was estimated at less than 10%. According to NCI SEER data, that rate now exceeds 90%.
This transformation happened through clinical research. Researchers systematically tested different chemotherapy combinations, different dosing schedules, different durations of treatment. Each trial built on the knowledge from previous trials. When a new approach showed promise, it became the new standard, and the next trial tested whether it could be improved further. Over decades, this iterative process of hypothesis, testing, refinement, and retesting produced the treatments we have today.
This is what clinical research does. It takes uncertainty and, through systematic inquiry, transforms it into knowledge. It takes hope and, through rigorous testing, determines whether that hope is justified. It takes individual stories and, through aggregation and analysis, reveals the patterns that apply to everyone.
The knowledge generated by clinical research does not stay locked in academic journals. It flows into clinical guidelines, medical school curricula, hospital protocols, and eventually into the exam room where a physician sits with a frightened parent. That parent is desperate for hope. What the physician can offer is something better than hope: evidence. Not certainty, because medicine is never certain. But the most reliable knowledge available about what treatment gives their child the best chance.
What evidence cannot tell us
Intellectual honesty requires acknowledging that evidence-based medicine has limitations. Clinical research tells us what works on average, but every patient is an individual. A treatment that benefits most patients might harm some. A treatment that shows no average effect might help specific subgroups.
Evidence also takes time to accumulate. When a new disease emerges, as happened with COVID-19, physicians must act before definitive evidence exists. Clinical judgment, informed by relevant prior knowledge and basic biological principles, remains essential when evidence is incomplete.
Furthermore, clinical trials often study narrowly defined populations: younger, healthier, more compliant patients than typical practice encounters. Whether trial results apply to a specific patient requires judgment. A treatment proven effective in 50-year-olds may or may not work in 80-year-olds with multiple other conditions.
Finally, evidence alone cannot make decisions. It informs decisions by clarifying what is likely to happen under different treatment choices. But patients have different values, different priorities, and different tolerances for risk. Evidence-based medicine at its best combines the best available evidence with clinical expertise and patient values to arrive at decisions that serve the individual patient.
Evidence tells us what is likely to happen under different choices. It cannot tell us what we should do, because that depends on what the patient values.
For example, evidence might show that an aggressive cancer treatment offers a 20% chance of cure but causes severe side effects in most patients. A younger patient might choose treatment for that chance at many more years. An elderly patient might prefer comfort and quality of remaining time. Both decisions can be consistent with the same evidence because they reflect different values.
Clinical research generates the evidence. Patients, guided by their physicians, decide what that evidence means for their lives.
